Background: N-terminal proB-type natriuretic peptide (NT-proBNP), high-sensitivity C-reactive protein (hs-CRP), and cardiac troponin-I (Tn-I) are good cardiac biomarkers in prediction of poor outcome in patients with acute coronary syndrome. The aim of this study was to assess the prognostic value of these cardiac biomarkers in patients with ST-elevation myocardial infarction (STEMI) treated with drug-eluting stent (DES) implantation. Methods and Results: The baseline levels of NT-proBNP, hs-CRP, and Tn-I were measured in 713 patients (528 males; 63짹12 years) with acute STEMI who underwent primary percutaneous coronary intervention with DES placement. Composite end point of major adverse cardiac events (MACEs) was assessed after six months. There were 53 MACEs: cardiac death: 5, myocardial infarction: 2, repeat PCI: 45, need for coronary artery bypass grafting: 1. Baseline levels of NT-ProBNP were 40.5짹21.1 pg/mL, 197.7짹89.4 pg/mL, 957.0짹374.6 pg/mL, and 5742.5짹6096.4 pg/mL in the lowest, the second, the third, and the fourth quartiles, respectively. Baseline levels of hs-CRP were 0.38짹5.43 mg/L, 0.50짹0.14 mg/L, 1.57짹0.70 mg/L, and 48.15짹168.80 mg/L in the lowest, the second, the third, and the fourth quartiles, respectively. The higher the levels of NT-proBNP and hs-CRP, the higher were the risk of poor long-term clinical outcomes. Compared with the lowest quartile, patients in the fourth quartile had a higher risk of MACEs: the relative risk was 1.42 and 2.39 (p=0.036), respectively. In contrast to NT-proBNP and hs-CRP, serum values of Tn-I were not associated with the risk of 6-month MACEs. Conclusion: Combined use of NT-proBNP and hs-CRP has incremental value in long-term risk prediction of cardiac events in patients with STEMI who underwent DES implantation.
