Purpose: In patients (pts) with aplastic anemia, chronic iron overload due to multiple transfusion can lead to the cardiac hemochromatosis. The aim of this study was to elucidate whether deferasirox, iron-chelating agent, can improve the cardiac function and reverse the cardiac dysfunction in pts with aplastic anemia. Methods: 32 pts (age 32짹10, M:F=17:15) with aplastic anemia who had received multiple blood transfusion during the course of disease were recruited. All pts were treated with deferasirox for 50 weeks. The serum ferritin level was measured before and after the treatment. Echocardiography was performed before, at 25th week and after treatment. The echocardiographic parameters including end-diastolic and end-systolic volume of left ventricle (LV), LV EF (%), LV and RV wall thickness, LA volume and LV filling indexes were measured. The echoreflectiveness of papillary muscles using histogram were quantified. Results: 1. The serum ferritin level was decreased from 4727짹2796.5 ���/L to 3176.7짹2635.1���/L after deferoxamine treatment (P<0.0001). 2. LVEF in 4 pts (12.5%) who showed decreased LV systolic function before treatment was improved from 52.1짹0.9 % to 56.3짹1.5 % (p<0.05) after treatment. 3. LA volume in 14 pts (44%) was decreased from 60짹19 mL to 54짹17 mL after treatment.(p<0.05) 4. E/E��� in 11 pts (34.4%) was decreased from 8.2짹1.4 to 7.5짹1.0 (p<0.05). 5. LV mass in 16 pts(50%) was decreased from 81짹13 gr/m2 to 73짹12 gr/m2 (p<0.05) 6. In 4 pts with EF竊�55% initially, echo-reflectiveness was decreased from 227짹6 to 205짹16 after treatment (p<0.05) Conclusion: In chronic iron overload, short-term treatment with iron chelating agent can improve the cardiac function and reverse the cardiac dysfunction. In these pts, long-term follow-up should be needed.
