Background:Uric acid(UA)level has been associated with an increased risk of cardiovascular events. However,it is unclear whether UA can provide prognostic information independent of conventional risk factors in severe heart failure with idiopathic dilated cardiomyopathy(IDCM).Method:61 patients with IDCM(age 63짹14years,37 males)were included ,evaluated with echocardiography.UA and N-terminal pro B-type natriuretic peptide(NT-proBNP)level were measured. clinical events were defined as the composite of cardiac death, admission for recurrent heart failure, while the patients were followed up.Results: During follow-up(mean 280 days),of all the patients(mean ejection fraction(EF)27%),13 cardiac events(1 death,12recurrent heart failure).no significant differences between patients with events and without events as regards age, diabetes, NT-proBNP and EF. In the patients with events, who had a trend towards more frequency of hypertension and history of heart failure, UA level was significantly higher(9.6짹2.4 vs. 6.4짹2.6 mg/dL;p<0.001)left atrial was more enlarged(5.0짹0.7vs. 4.5짹0.5cm;p=0.008), UA did not correlate with NT-proBNP(r=0.122,p=0.354).high UA level(hazard ratio, 3.13;p=0.018)and left atrial dimension (hazard ratio,26.5; p0.044) were shown to be independent predictors for cardiac events by Cox regression analysis.Conclusion:High UA level could be a prognostic predictor in patients with IDCM,irrespective of NT-proBNP and suggest the combination of UA level combined with left atrial dimension would be useful for prediction of cardiac events.
