Changes of Regional Right Ventricular Function during Hemodialysis: A Quantitative Echocardiographic Study in Patients with End Stage Renal Disease |

Background: Recent studies showed that changes in loading conditions did not affect the regional right ventricular (RV) strain and strain rate (ε/SR). The aim of this study was to evaluate the response of RV tissue velocities and ε/SR during hemodialysis (HD) in which both significant preload reduction and complex hemodynamic changes occur.
Methods: Standard echocardiography with Doppler tissue imaging was performed within 1-h before and after HD. Tricuspid annular plane systolic excursion (TAPSE) was obtained as an estimate of RV global function. Percent change (Δ%) was calculated as [(after HD – before HD)*100] / after HD.
Results: A total of 30 patients (16 men; mean age, 51.6 ± 13.3 years) were enrolled. Estimates for global and regional RV function were decreased significantly after HD (Table). Δ% of tricuspid annulus e'velocity was correlated with ultrafiltration volume (UF) (r = 0.481, p=0.006). Δ% of mid RV systolic (r = 0.421, p=0.018), early (r = 0.423, p=0.018) and late diastolic (r = 0.410, p=0.022) tissue velocities were correlated with UF. Δ% of mid RV ε/SR did not correlate with UF but Δ% of peak systolic SR and peak strain was correlated with age.
Conclusion: Regional RV ε/SR was changed significantly after HD but the degree of changes was not related with volume reduction. This finding may suggest that multiple factors other than volume reduction affect the regional RV function during HD in patients with ESRD.
