Coronary Risk Control Evaluated by Health Screening in Local Community

Background : Risk factor interventions and health education programs to prevent coronary artery disease (CAD) are effective way reducing the prevalence of coronary heart disease. It’s well known that relative to current guidelines, rate of risk factor control is very low. We have analyzed the rate of two known two modifiable coronary risk factors, hypertension and dyslipidemia, in the community.Methods : A total of 456 participants (20-86 years, 256 males) to a health educational program in Daegu city were enrolled in this study. Risk factor knowledge, smoking and medication histories were determined by questionnaire. Body weight, height, blood pressure, and lipid levels were recorded. Blood pressure and lipid control rates were evaluated based on the JNC-7 and NCEP lipid guidelines. Results : The average 10-year CAD risk of the participants were 7.2% , and 64.5% were having more than 2 CAD risk factors. Most frequent risk factors were age, hypertension and low HDL-cholesterol level. The prevalence of hypertension was 56.4% , 176 of the 257 hypertensive patients (68.5%) were aware of their hypertension and taking medications. Among all hypertensive participants, BP was controlled in only 40.0%. Mean lipid levels were as follow; total cholesterol 183±35, LDL-cholesterol 101±32, HDL-cholesterol 45±11, triglyceride 192±122 mg/dL. Among all participants, 7.5% were taking lipid-lowing agents and 87.3% were controlled their LDL-choleterol levels under the guideline. Inadequate lipid control rate was much higher in high risk patients (12.7%) than moderate (5.9%) or lower risk patients (1.5%) (p<0.01). Conclusions : As reported in Western country, optimal blood control rate is very low in Korea. And we need more efforts controlling dyslipidemia in high risk patients.
