Comparison of Nonuniform Strut Distribution among Three Drug-Eluting Stent Platforms

Background: Nonuniform strut distribution (NSD) has been shown to be associated with increased intimal hyperplasia after drug-eluting stent(DES) implantation. Previous study revealed NSD was less in Cypher stents than in Taxus Express stents. There is little data regarding the NSD of the new DES platform (Taxus liberte). The aim of this study was to compare the difference of strut distribution among three clinically available drug-eluting stents (Cypher select, Taxus liberte and Endeavor stents) using intravascular ultrasound (IVUS). Methods: NSD was defined as interstrut angle > 90°, and NSD segment was defined as any segment observed for more than 0.5 mm longitudinally. Percent NSD was defined as the length of segments with NSD divided by stent length. NSD was assessed in post procedure IVUS images in 62 clinical cases (24 Cypher select, 24 Taxus liberte and 14 Endeavor stents) using 3-dimensional IVUS analysis. Results: Percent NSD was more in Eneavor but did not reached statistical significance (Cypher select: 0.54% vs Taxus liberte: 0.578% vs Endeavor Rx: 1.56%, p= 0.231) and frequency of NSD segment was more in Endeavor but did not reached statistical significance (Cypher select: 12.5% vs Taxus liberte: 12.5% vs Endeavor: 21.4%, P=0.333). Conclusions: Our data shows that there is no difference in NSD between three clinically available DES. The NSD of the new DES platform (Taxus liberte) is similar with Cypher stent.
