Background: Aortic stiffening contributes to the LV afterload, hypertrophy and substrate for diastolic dysfunction. It is also known that aortic elastic properties could be investigated with colour tissue dopper imaging (TDI) in aortic upper wall. Therefore we would like to seek to relation of aortic wall TDI and aortic stiffness and other parameters of diastolic dysfunction. Method: We examined aortic upper wall of 500 patients by TDI at the 3cm above the aortic valve. We measured aortic diameter at systolic, diastolic phase and the velocity of TDI in the aortic wall, mitral annulus (Em, Am, Sm) and PW of E, A, DT in the mitral inflow. We excluded the patient with arterial hypertension or reduced LV ejection fraction or significant valvular heart disease. So a total of 126 (mean age 53.8짹13.9, male 49.2%) patient were enrolled in this study and divided normal LV filling group (N=31) and abnormal LV filling group (N=95) depending on E, A, DT, Em, Am, Sm. Blood pressure were measured in the brachial artery before the echocardiogram. Result: Aortic wall early and late systolic velocity and late diastolic velocity were not different between two groups. Only Aortic wall early diastolic velocity was significantly reduced in abnormal LV filling group and related to aortic stiffness index (r=-0.25, P=0.008), distensibility (r=0.27, P=0.003), Em (mitral annular velocity in early diastole r=0.448, P=0.001), E/Em (r=-0.25, P=0.005) Conclusion: Aortic wall early diastolic velocity is decreased significantly in abnormal LV filling patient. And it is significantly related to aortic stiffness, distensibility and abnormal LV filling, Em, E/Em. TDI of the aortic wall can be a new tool for aortic stiffness, distension and diastolic function.
