The feasibility and determinant of heart-femoral pulse wave velocity.

Objective: a simple method of the heart-femoral PWV (hfPWV) and brachial-ankle PWV (baPWV) are recently available, even though carotid-femoral pulse wave velocity (PWV) is a standard method for measuring PWV. However the feasibility of hfPWV has not been fully determined. The aim of this study was to evaluate the feasibility of hfPWV and correlation between hfPWV and baPWV in patient with diabetes mellitus (DM) or hypertension (HTN) and without DM and HTN. Material and methods: patients with DM or HTN (Group I, n=141, 76 men, 58 ± 9 years) and patients without DM and HTN (Group II, n=51, 36 men, 54 ± 10 years) who undergone testing a baPWV were included. The measurement of the baPWV were performed in a quiet controlled room (22 ± 1 °C) in the supine position with using an automated device (VP-2000, Colin, Co. Ltd, Komaki, Japan). Results: Group I was higher hfPWV and baPWV than Group II (1077 ± 237 cm/sec vs. 934 ± 191 cm/sec, p<0.001 and 1715 ± 337 cm/sec vs. 1496 ± 305 cm/sec, p<0.001). Patients with DM was higher hfPWV and baPWV than patients without DM (1114 ± 256 cm/sec vs. 994 ± 207 cm/sec, p<0.001 and 1757 ± 379 cm/sec vs. 1598 ± 304 cm/sec, p<0.01). patients with HTN was higher hfPWV and baPWV than patients without HTN (1083 ± 229 cm/sec vs. 993 ± 230 cm/sec, p<0.01 and 1721 ± 314 cm/sec vs. 1577 ± 360 cm/sec, p<0.01). hfPWV was positively correlated with age (r=0.469, p<0.01), baPWV (r=0.740, p<0.01) and systolic blood pressure (r=0.397, p<0.01) and multivariate linear regression analysis showed significant association of hfPWV with age (β=9.5, p<0.001), systolic blood pressure (β=4.1, p<0.001) and HDL-cholesterol (β=-3.7, p<0.001), R2=0.366 in all study subjects. Conclusion: in this study, hfPWV was higher in patients with DM and HTN than patients without DM and HTN and this results suggested that age, systolic blood pressure and HDL-cholesterol play a significant role in the arterial stiffness and also hfPWV might be feasible for measuring PWV.
