HDL Cholesterol is a Powerful Predictor of Coronary Heart Disease

Objective ; Low-density lipoprotein cholesterol(LDL-c) contributes a major risk factor for premature development of coronary heart disease(CHD) and the main target for therapy. However, CHD is still the leading cause of death in many countries. Growing evidence suggest that high-density lipoprotein cholesterol(HDL-c) is a powerful predictor of CHD. The aim of this study is to evaluate the relations between lipoprotein level, lipoprotein ratios and the development of CHD. Material and methods ; 485 patients were divided into two groups. All subjects did not take antilipidemic agents. Group 1(n=432) did not have the any evidence of CHD and also not have carotid plaque by ultrasound.Group 2 was CHD patients who were definitely diagnosed(n=53). Blood sample was collected from each subject after 12 hour fast. Results; The subjects in the two groups were mainly man(53.7% vs 56.6%) with high portion of hypertension(54.2% vs 43.4%) and diabetes(50.6% vs 52.8%). LDL-c(108.7±33.0mg/dL vs 110.5±34.6mg/dL, p=0.72), triglyceride(136.4±91.4mg/dL vs 138.5±74.7mg/dL, p=0.87), non-HDL-c(132.1±36.5mg/dL vs 136.0±36.4mg/dL, p=0.46), hsCRP, body mass index, uric acid were not significantly defferent between two groups. The age(56.7±10.7 vs 65.3±10.3, p=0.00), HDL-c(49.1±13.3mg/dL vs 40.4±13.0mg/dL, p=0.00), Total cholesterol/HDL-c ratio(4.0±1.4 vs 4.7±1.5 p=0.00), LDL-c/HDL-c ratio(2.4±0.9 vs 2.9±1.1, p=0.00), smoking(n=75(17.8%), n=17(32.1%), p=0.01) were significantly different. In the multivariate linear regression, age(odds ratio(OR) 1.08, p=0.00) HDL-c(OR 0.96, p=0.01), smoking(OR 2.89, p=0.00) were significantly associated with CHD development. Conclusion; HDL-c is a strong predictor of CHD among lipoproteins and the control of HDL-c will be one of the important issue in the future.
