Purpose: This study was aimed to investigate the usefulness of contrast echocardiography in prediction of successful reperfusion after percutaneous coronary intervention (PCI) for chronic total occlusion (CTO) lesion. Method: 54 myocardial segments of 15 patients (mean age=63.0짹14.9 years, 9 males) with documented CTO lesion in coronary angiography were divided into two groups according to results of intervention : Group I (segments that successfully reperfused with PCI, n=26segment) and group II (segments that fail to reperfuse with PCI, n=28segment) . Before PCI, peak intensities (慣) and rate parameter (棺) of contrast, wall motion score (WMS) were measured in contrast echocardiographic images and compared between two groups. Results: Peak intensities (慣) of contrast in involved myocardial segment were significantly higher in group I than in group II (5.76짹2.74 vs. 3.96짹2.47, p=0.015). Rate parameter (棺) of contrast in involved myocardium (0.088짹1.55 vs. 2.63짹10.47, p=0.389) and WMS (2.04짹0.662 vs. 1.68짹0.819, p=0.083) were not different between two group. On receiver operation curve analysis, peak intensity (慣) showed powerful diagnostic power to predict successful PCI for CTO lesion (area under the curve=0.769). The optimal cutoff value of peak intensity (慣) was 3.58 (sensitivity: 76.9%, specificity: 57.1%). Conclusion : This study results suggested that peak intensity of involved myocardial segments measured by contrast echocardiography before coronary intervention is useful for predict favorable results in CTO lesion.
