The Comparison of Clinical Outcomes for Infarct Related Artery Revascularization only and Multivessel revascularization in the Patients with Acute Myocardial Infarction Combined with Cardiogenic Shock

The aim of the study was to assess the impact of mutiple revascularization for non-infarct and infarct related arteries in patients with acute myocardial infarction (AMI) combined with hemodynamic compromise during the hospitalization. We analyzed the Korean Acute Myocardial Infarction Registry (n=7,673 consecutive patients with AMI who had undergone PCI from November 2005 to January 2008) and we selected the patients who showed cardiogenic shock and multivessel disease with AMI. Of the patients (n=354), Major adverse cardiac events (MACE) such as cardiac death, nonfatal MI and repeat revascularization were compared between the patients with multivessel revascularization (n=139) and with revascularization for infarct-related artery only (n=215) at 1 month and 12 months after PCI. There were no significant differences in the clinical and angiographic characteristics except target vessel (P=0.025). Total MACE rate at 1 year were similar between 2 groups (P=0.292). However, the group with multivessel PCI had higher rate of target vessel revascularization and target lesion revascularization at 12 months (5.0% vs 1.4%, P=0.043). These data suggest that performing multivessel PCI for the patients with AMI and cardiogenic shock during the hospitalization does not improve 1 year survival rate. and even increase the rate of revascularization.
