ECG characteristics and classification with PVCs in patients without ischemic heart diseases

Introduction: PVCs(PVCs) are common and relatively insignificant. However, we could not say that all PVCs are benign. One cause of PVCs may be myocardial ischemia. If so, what are the characteristics of non-ischemic PVCs? Methods: We reviewed all consecutive standard 12 leads ECGs from January 2010 to June 2010 in our center. Totally 6554 patients underwent an ECG exam. We reviewed clinical, echocardiographic and ECG characteristics in patients without ischemic heart diseases. Results: Among total patients, 402 patients had PVCs with sinus rhythm. Among them, 305 patients did not have any history of coronary artery diseases. Mean age was 59 ± 16.0 years (135 females, 44.3%). Incidence of hypertension, diabetes mellitus, dyslipidemia, smoking and cerebrovascular accidents were 35.1%, 15.4%, 1.0%, 21.6% and 4.6% respectively. One hundred patients (32.8%) complained symptoms. Among them, 66 patients (66%) received medical treatments. Among them, beta blockers were prescribed to 44 patients (66.6%). Left ventricular ejection fraction was 59.1 ± 13.0 %. Left ventricular end diastolic diameter was 51.3 ± 6.5 mm. Left atrial diameter was 37.6 ± 6.4 mm. Seven point five percent of patients had moderate to severe mitral valve regurgitation and 2.9% of patients had moderate to severe tricuspid valve regurgitation. Right bundle branch block(RBBB) pattern was 76.7% and left bundle branch block(LBBB) pattern was 23.3%. Proportion of isolated PVCs, PVCs bigeminy, trigeminy, quardrigeminy and interpolated PVCs were 80.0%, 8.5%, 6.9%, 2.3% and 2.6% respectively. T wave axis of normal sinus rhythm was different between RBBB and LBBB group(40 ± 34.7 vs. 56 ± 39.7, p = 0.001). In RBBB pattern, the percentage of south-east, south-west, north-west and north-east axis were 35%, 30.3%, 9.4% and 23.9% respectively. In LBBB pattern, the percentage of south-east, south-west, north-west and north-east axis were 39.4%, 35.2%, 5.6% and 16.9% respectively. PVCs from right ventricular outflow tract was 17.7%. Bigeminy PVCs were more frequent in LBBB pattern (5.9% vs. 16.9%, p = 0.024). Interpolated PVCs were more frequent in RBBB pattern (3.4% vs. 0%, p = 0.004). There was no difference of symptom incidence between north-west axis and non-north-west axis. In north-west axised PVCs, PVC width and PVC number in 10 seconds were larger than non-north-west axised PVCs (144 ± 25.5 ms vs. 132 ± 25.6 ms, p = 0.029; 2.0 ± 1.62 vs. 1.4 ± 0.98, p = 0.004). Conclusion: We could not define that all PVCs are benign. There was no symptomatic difference between bundle branch block pattern of PVCs. We should pay more attention to PVCs with north-west axis in patients without ischemic heart diseases.
