Objective: Hybrid procedure with endovascular stent provides an extended treatment options for aortic disease of elderly patient. It is useful options for high risk patients who have contraindications of classic endovascular repair. We report the early results of Hybrid endovascular stent insertion for aortic diseases.
Methods : Between May 2008 and July 2010, Hybrid endovascular stent insertion was performed in twelve male patients who had contraindications of endovascular stent or high risk of complication. The mean age was 75.8 ± 8.3 years. We performed several hybrid techniques such as 3 kinds of off-pump arch vessels debranching for distal arch involved aneurysms, 7 femoro-femoral bypass for abdominal aneurysms, 2 peripheral artery bypass for aortoiliac occlusive disease (7 in the operating room, 5 in the angiographic room). The results were assessed before discharge with contrast-enhanded CT.
Results : One patient with thoracic aneurysm died on the day of surgery because of technical and climical stent failure. No major or minor complications occured, except newly developed claudication in one patient. An endoleak was seen on postoperative CT in three patients(1Type Ⅰ, 2type Ⅲ).
Conclusions : Hybrid endovascular stent insertion is relatively safe and effective procedure for high risk patients who have contraindication of classic endovasscular repair. But it seems that its indications and its long term results need to be further assessed.
