Comparison of 6-month and 9-month follow up in everolimus-eluting stent: evaluated by optical coherence tomography

Background Delayed neointimal coverage is a major concern to late stent thrombosis after stent implantation. The serial change of neointimal coverage after everolimus-eliuting stent was evaluated using optical coherence tomography (OCT). Methods Total 20 patients who had everolimus-eluting stent implantation underwent OCT at 6-month (10 patients) and 9-month (10 patients) after stent implantation. The thickness of neointima and percent of neointimal hyperplasia (NIH) on each 1 mm strut cross section and strut apposition to the to the vessel wall were evaluated. The stent strut was classified into 4 category; well-apposed to vessel wall with apparent neointimal coverage (A), well-apposed to vessel wall without neointimal coverage (B), malapposed to the vessel wall with neointimal coverage (C), malapposed to vessel wall without neointimal coverage (D). Results Total 5133 struts (2613 struts for 6-month, 2520 struts for 9-month) were evaluated. The mean NIH thickness (69.5 ± 91.1㎛ vs. 108.3 ± 523.5㎛, p<0.001) and percentage of NIH area (6.59 ± 4.96% vs. 13.57 ± 6.76%, p<0.001) were significantly greater at 9-month than 6-months follow up. The prevalence of uncovered strut (well-apposed and malapposed) was significantly lower at 9-month follow up (2.83 ± 8.58% vs. 27.61 ± 31.02%, p<0.001). Type A category coverage was significantly greater at 9-month follow up (96.12 ± 11.8 vs. 69.70 ± 32.69%, p<0.001). The percentage of well-neointimal coverage in malapposed strut was not statistically different at two follow up period (50 ± 57.74% at 9-month vs. 30.32 ± 35.34% at 6-month, p=0.549). Two patients of 6-month follow up and one patient of 9-month follow up were observed in-stent thrombosis. Conclusions Neointimal coverage at 9-month was almost complete in everolimus-eluting stent. Noeintimal coverage in malapposed strut was not different between two follow up periods.
