Ophthalmic Artery Doppler Flow Predicts the Severity of Coronary Artery Disease

Background : The ophthalmic artery (OA) is the first major branch of the internal carotid artery and OA Doppler has anatomical advantage due to the absence of ultrasonic obstacles and the good sonolucency of the eye-ball, and the vertical angle of OA to the transducer. We evaluated clinical usefulness of Doppler waveform indices of OA for estimating the severity of coronary artery disease(CAD). Methods : We enrolled 36 patients who were diagnosed CAD by coronary angiography (1 Vessel Disease:19, Multi-Vessel Diease:17) and 22 normal control who had no evidence of CAD. Revascularized coronary arteries were considered as diseased vessels. After sterile ophthalmic gel was applied to the closed eyelid, the blood flow velocity of OA was assessed with pulsed Doppler. Several indices of pulsed Doppler, such as peak systolic velocity (PSV), end-diastolic velocity (EDV), mean systolic velocity (Sm), mean diastolic velocity (Dm), and mean total velocity (Tm) were measured and pulsatile index (PI) and resistance index (RI) were calculated by using these indices. Results: PSV of both OA were significantly lower in patients with CAD than in controls (Left, control : 35.09±11.97, 1VD : 29.05±8.3, MVD : 27.44±5.36 cm/s, P=0.031, Right, control : 36.36±11.96, 1VD : 29.11±5.6, MVD : 30.06±6.03 cm /s, P=0.019). The levels of Sm/Dm were higher in patients with CAD but were not statistically significant. There were no significant differences between EDV, Sm, Dm, PI and RI. Conclusions: The Doppler flow of OA could predict the severity of CAD and could be used as another noninvasive tool for evaluating the severity of atherosclerosis and CAD.
