High Density Lipoprotein Cholesterol Level Can Predict the Revascularization after Percutaneous Coronary Intervention |

Purpose: High-density lipoprotein cholesterol (HDL-C) level is a strong inverse predictor of cardiovascular events. We investigated the significance of HDL-C level on the revascularization of patients who underwent percutaneous coronary intervention (PCI) with drug eluting stent (DES).
Method: Among 6,749 consecutive patients who underwent PCI with DES form January 2004 to December 2009 in the COACT (CathOlic medical center percutAneous Coronary intervention) Registry, we analyzed follow up (f/u) lipid panel of 3,076 patients (mean lab f/u duration 283 ± 249 days) and compare clinical outcomes according to f/u HDL-C level. Low HDL-C level was defined as HDL-C level under 40 mg per deciliter for men or 50 mg per deciliter for women. The primary end point was a composite of any revascularization.
Results: Mean follow-up duration was 785 ± 516 days. 1,731 (56.3%) patients had low HDL-C level and 1,345 patients had high HDL-C level. High HDL-C didn’t affect the incidence of cardiac death and acute myocardial infarction after multivariate Cox analysis. High HDL-C was a significant independent inverse predictor of total revascularization (adjusted hazard ratio 0.777, 95% confidence interval 0.630 to 0.960, p=0.019).
Conclusion: HDL-C level is associated with total revascularization in patients who underwent PCI with DES, introducing the next therapeutic target for coronary artery disease.
