Predictors Can Improve the Angina Symptom in Patients with Variant Angina.

Purpose: We don’t know well the predictors on the improvement of angina symptoms in patients with variant angina. Method: Among 740 patients with variant angina diagnosed by coronary angiogram in the Variant Angina Catholic Medical Center Registry, we compared the initial and 1 year follow up angina symptom of 548 patients by grading the frequency of angina symptom. We defined the grade of angina symptom; 1 – almost daily (22.1%), 2 - more than once a week (19.2%), 3 - more than once a month (28.5%), 4 - less than once a month (30.3%). Results: Mean age of patients was 58.61 ± 10.79 years old. Male gender was 49.5% (n=271). The patients with improvement of angina symptom was 59.9% (n=328). The characteristics of patients with symptom improvement are listed; younger age, more male gender, more past history of coronary artery disease, less past history of dyslipidemia, more family history of coronary artery disease, more current smoker, more alcohol drinker, more multi-vessel spasm, more use of long acting nitrates and more nicorandil. The use of calcium channel blocker (CCB) was not different between the patients with and without symptom improvement. The factors independently affected the improvement of angina symptom was past history of CAD, past history of dyslipidemia, alcohol drinking, and multi-vessel spasm. The medications such as CCB did not affect the improvement of angina symptom. Conclusions: Many extrinsic factors such as past history of CAD and dyslipidemia, and alcohol drinking independently affected the improvement of symptom in patients with variant angina. Correcting many extrinsic factors might add additional effect to improve angina symptoms.
