Long-term Outcomes in Octogenarians Undergoing Percutaneous Coronary Intervention with Drug-Eluting Stents

Background: There was a paucity of data about clinical outcomes of octogenarian with percutaneous coronary intervention (PCI) because of excluding octogenarian in most of randomized controlled trial. So the aim of the present study is to assess the clinical characteristics and outcomes of octogenarian with PCI compared with non-octogenarian. Methods: Among consecutive 6749 patients performed PCI in COACT (CathOlic medical center percutAneous Coronary inTervention) registry from January 2004 to December 2009, we analyzed 4707 patients with stable or unstable angina who underwent PCI with drug eluting stents. We divided them into two groups according to their age (≥80 and <80) and compared the clinical outcomes. The primary clinical outcome was a composite of death, MI. Results: Two hundred forty three patients were octogenarians (5.2%). There were more hypertension, more renal insufficiency, more anemia, more female gender, less current smoker, less hypercholesterolemia, and less obese in octogenarian. Cox hazard regression analysis demonstrated that octogenarians were more prone to composite death and myocardial infarction (adjusted hazard ratio 1.54, 95% confident intervals 1.23-1.91, p<0.001). However, target lesion revascularization occurred statistically less in octogenarians (adjusted hazard ratio 0.594, 95% confident intervals 0.370-0.955, p<0.031) (Table).Conclusion: Long term mortality in octogenarians is significantly higher than younger patients but repeat revascularization rate is lower than younger patients.
