Pediatric Tachyarrythmia and Radiofrequency Catheter Ablation: Results from 1993 to 2011

Purpose: Radiofrequency catheter ablation (RFCA) is widely accepted as a curative treatment for many types of cardiac arrhythmias. It has recently become a management option for pediatric tachyarrythmia. The aim of this study is to provide the results of RFCA for SVT in children and adolescents in Daegu. Methods: The authors retrospectively reviewed the records of a total of 260 patients (aged 2 to 18 yr) who had undergone RFCA from August 1993 to July 2011 at two medical centers in Daegu. Results: 260 of 1524 patients (17%) underwent EPS and RFCA at an age less than 18 years. Mean number of patients per year was 13. Of 260 patients, 9 patients (3%) were younger than 6-year-old, 76 patients (30%) were between 7- and 12-year-old, and 175 patients (67%) were aged more than 12. There were 157 males and 103 females. Types of arrhythmia were AVRT with CBT (94/273, 35%), AVRT WPW (81/273, 30%), AVNRT (83/273, 30%), AT (2/273, 0.7%), mahaim fiber (1/273, 0.3%), VT (12/273, 4%). Ablations of AVRT were the most common procedures (175/273, 64%). 77% (72/94) CBT were located at the left side and 59% (48/81) WPW were located at the right side. 66% (55/83) AVNRT were located at the M1 and/or M2 site. 75% (9/12) VT were located at the left mid-inferoseptum. Thirteen patients (5%) had dual pathway. They showed 3 AVNRT+CBT, 1 AVNRT+WPW, 3 CBT+WPW, 3 dual CBT, and 3 dual WPW. Associated cardiac disease was detected in 5 patients including 2 univentricular patients, 1 Ebstein’s anomaly patient, 1 ASD, 1 MVP. They showed 3 left side CBT, 1 right side CBT, and 1 right side WPW. Seventeen patients were recurred and the recurrence rate was 6.5%. Of these patients, 16 patients had AVRT with CBT or WPW and 7 patients showed left side AP. Failure of RFCA was occurred in 14 patients and 64% (9/14) were right side AP. Patients with AVNRT did not recur or fail. Only one patient, an 8 year-old girl, developed a CAVB after RFCA for AVNRT. Overall success rate was 94.7%. Conclusion: RFCA of cardiac arrhythmia in our institutions are safe and effective in the management of pediatric tachycardia patients.
