Impact of Alcohol Drinking Pattern on Clinical and Angiographic Characteristics of Coronary Artery Spasm

Background: There have been very limited data regarding the impact of alcohol consumption amount or pattern on the clinical and angiographic characteristics of coronary artery spasm (CAS). We evaluated the impact of alcohol drinking pattern on CAS as assessed with intracoronary acetylcholine (Ach) provocation test. Methods: A total 912 consecutive alcoholic patients [pts, Men, 698 (76.5%), mean age 50.56±11.8 years] who underwent coronary angiography and Ach provocation test were enrolled. Study population were divided into 2 groups; light to moderate alcoholic defined as average daily consumption of ethanol less than 30g/d whereas heavy alcoholic consume more than 30g/d . Results: Baseline clinical characteristics were similar between the two groups except that heavy alcoholic group had more men (95.1% vs.74%, P<0.001), smokers (55.6% vs. 40.5%, P<0.001), hypertension (48.6% vs.38.4%, P=0.023), diabetes (16.7% vs.9.4%, P=0.011), whereas moderate alcoholic group had more dyslipidemia (20.2% vs. 8.3%, P=0.001). Results of Ach provocation test were similar between the two groups except for heavy alcoholic had less occurrence of ischemic chest pain during the test but showed higher rate of significant response to lower Ach dose in univariate analysis. After adjusting the baseline differences, all endpoints of Ach provocation test were not different between the two groups (Table). Conclusions: In our study, there was no difference in clinical and angiographic parameters during the Ach provocation test between light to moderate and heavy alcoholics, suggesting limited role of alcohol consumption amount on CAS.
