The angiographic characteristics of vasospastic angina in Korea : The results of Vasospastic Angina in Korea (VA-KOREA) registry

Backgound: Vasospastic angina (VA) has been reported to be more frequent in the East than in the West. However, in Korea, a grand scale analysis has not been done yet. We planned to set the multi-center registry for survey about current practices, prognosis, and racial differences of Korean VA. Methods: Total 11 centers of 9 universities participated in web-based VA-KOREA registry and enrollment of patients newly diagnosed as VA using same diagnostic protocol started in July 2010. We only used intracoronary injection of ergonovine, not acetylcholine, in provocation. Positive response for the provocation is defined as development of total or subtotal occlusion accompanied by angina and/or ECG changes. We got information about medical history, risk factors, laboratory, and angiographic findings, treatment and prognostic factors. Study end point is the major adverse cardiovascular events during 3 year-follow up. First, we analyzed angiographic characteristics using data of VA-KOREA registry. Results: Total 1105 patients were enrolled to the registry. 315 patients (28.5%) demonstrated definite positive result on provocation and 540 (48.9%) showed negative result. Other 250 patients had intermediate vasoconstriction (defined as “possible” or “probable”). In analysis for 315 “positive”, multi-vessel spasm occurred in 50 patients (15.9%) and 265 (84.1%) showed single vessel spasm. Right coronary artery (RCA) was the most common epicardial artery for single vessel vasospasm (151/265, 56.9%). The simultaneous vasospasm on left anterior descending and left circumflex arteries was the most frequent type of multi-vessel vasospasm (20/50, 40.0%). Diffuse spasm type is more frequent than focal type (85.7% vs 14.3%). In 7 patients (2.2%), vasospasm was provoked on segment of underlying myocardial bridge. On baseline angiography, minimal fixed atherosclerosis (<50% of diameter narrowing) was found in 170 patients (54.0%). Out of these, 134 patients (78.8%) showed vasospasm on the segment with atherosclerosis. And the vasospasm was provoked on the segment without fixed stenosis in other 36 patients. 145 patients showed normal coronary artery on baseline angiography. Conclusions: The VA-KOREA registry will be helpful for establishment of standard guidelines for Korean VA patients. In preliminary angiographic analysis, the positive rate of ergonovine provocation was 28.5% and RCA was the most common artery for coronary vasospasm. Some cases were associated with underlying myocardial bridge. In cases of minimal fixed stenosis on baseline angiography, coronary vasospasm was more frequently provoked on the segment with atherosclerosis than on the segment without minimal stenosis.
