The experience of balloon assisted technique during percutaneous closure of atrial septal defect

Introduction: Percutaneous closure of atrial septal defect (ASD) has been shown to be safe and effective. However deficiency of the posterior and inferior rims or too large size of ASD has been shown to affect the anchorage of the device. Until now some techniques are introduced to overcome these problems. In this report, we describe our experience of using balloon assisted technique in 10 patients. Methods: Between January 2010 and August 2011, 94 patients underwent percutaneous closure of ASD and in 10 patents we used balloon assisted technique (BAT). We reviewed their medical report retrospectively and analyze their data with others. In this report we describe the technique and our experience of balloon assisted technique. Results: The mean device size was 32.6 ± 6.1 mm in BAT used group and it was 21.1 ± 6.3 mm in conventional method used group. BAT used group had longer procedure time (85.9 ± 20.7 min) than conventional method used group (69.4 ± 26.7 min). Our BAT was positioning inflated balloon through the ASD during device anchoring properly than deflation of the balloon was done. In five cases we used BAT from the beginning but in five cases we tried it after we failed device closure by conventional methods. In spite of using balloon assisted technique we failed in one case. Nine cases were large ASD (range, 27.3 ~ 38.05 mm) and one case had deficient posteroinferior rim. No major complications were noted during the procedure or short-term follow up period.Conclusion: The BAT can be used in the closure of large ASD or deficient posteroinferior rim without increasing any major complication. BAT prevents the device from slipping the floppy interatrial septum and from becoming perpendicular to interatreial septum. Therefore, appropriate use of BAT may increase the successful closure rate of difficult ASDs.
