Background: Despite of relatively lower incidence of culprit lesion restenosis in patients with acute myocardial infarcton (AMI) compared with those with stable angina, in-stent restenosis was still associated with poor clinical outcomes in AMI patients. Therefore, this study aimed to determine predictive factors of culprit lesion restenosis in AMI patients underwent stent implantation. Methods and Results: From February 2008 to August 2009, we analyzed the 276 AMI patients underwent successful stent implantation at index procedure and ischemic-driven or routine follow-up angiography after 9 months. During 9-month follow-up, cardiac death was occurred in 5 patients (1.8%), myocardial re-infarction in 6 patients (2.2%) and target lesion revascularization in 50 patients (18.1%). Of these, binary restenosis (percent diameter stenosis> 50%; BS) was observed in 49 patients (17.8%). Patients with BS were older (64.45 짹 12.52 vs. 59.72 짹 11.53, p=0.011), lower level of creatinine clearance (69.73 짹 35.39 vs. 87.76 짹 34.75, p=0.001) and more common in history of diabetes (42.9% vs. 21.6%, p=0.010), ischemic heart disease (18.4% vs. 8.4%, p=0.036) and use of bare metal stents (BMS)(34.7% vs. 11.5%, p<0.001), however, patients without BS in current smoker (28.6% vs. 48.9%, p=0.010) and prescription of statin at discharge (57.1% vs. 75.3%, p=0.010). Late loss were also higher in patients with BS (2.31 짹 0.74 vs. 0.24 짹 0.43, p<0.001). By multivariate logistic regression, the independent predictors for binary restenosis were use of BMS and prescription of statin at discharge [Hazard ratio (HR)=4.230; 95% Confidence interval (CI)=1.845-9.696, HR=0.403; 95% CI=0.203-0.799]. Conclusions; Our study shows that statin treatment may play an important role in reducing restenosis for patients with AMI underwent stent implantation.
