Adverse cerebrovascular events associated with transradial coronary intervention (Preliminary report from Korean TRIWG registry)

Backgrounds: Transradial intervention (TRI) for coronary artery disease is widely used due to both benefit in hemostasis and patients’convenience when compared to transfemoral intervention (TFI). Despite its wide use, cerebrovascular accident (CVA) associated with TRI is not well described. Methods: TRI working group involving 14 centers in Korea is registering all percutaneous coronary intervention (PCI) cases in 2009 irrespective of access route. From this incomplete registry at this time, we preliminarily analyzed the incidence of PCI associated CVA in patients whose access was exclusively transradial (including ulnar or brachial) or exclusively transfemoral. Results: The incidence of CVA was 0.5% (8 among 1497) in patients with TRI and 0.7% (14 among 1940) in patients with TFI (P=0.53). Non-hemorrhagic CVA occurred in 0.3% (5 among 1494) in patients with TRI and 0.6% (11 among 1937) in patients with TFI (P=0.45). Among TRI patients, the incidence of CVA was 0.2% (1 among 522) in patients with left TRI and 0.6% (6 among 976) in patients with right TRI (P=0.43). Non-hemorrhagic CVA occurred in 0.2% (1 among 522) in patients with left TRI and 0.4% (4 among 974) in patients with right TRI (P=0.66) Conclusion: There were non-significant trends towards lower incidence of total and non-hemorrhagic CVA in patients with TRI when compared to TFI, and with left TRI when compared to right TRI.
