Factors Influencing Depression in Adults with Congenital Heart Disease

Objectives: This study was designed to investigate the prevalence of depression in adults with congenital heart disease(CHD) and identify factors influencing the depression. Methods: The Center for Epidemiologic Studies-depression scale was administered in order to evaluate depression levels among 216 adults with CHD from a major cardiac center. This group consisted of 105 males and 111 females. The average age was 42.7 years(range: 18-68 years). Cardiac disease severity was operationalized in terms of initial diagnosis (classification of ‘Task force 1’ of the 32nd Bethesda conference), illness course(Disease severity index) and current functional status(NYHA class, ability index, congenital heart disease functional index, percutaneous oxygen saturation and plasma NT-proBNP). Data were analyzed using descriptive statistics, pearson correlation and multiple logistic regression with SPSS 19.0. Results: The average depression score of the subjects was 18.76, which indicates moderate levels of depression. About 33% of subjects in this study reported moderate or severe levels of depression. Significant predictors of depression were young age(OR=2.12, P<0.001), female(OR=1.89, P=0.012), worse NYHA class(OR=1.26, P=0.023), worse ability index(OR=0.69, P=0.017) and higher congenital functional index(OR=1.03, P=0.001), lower oxygen saturation(OR=1.56, P=0.023) and higher NT proBNP(OR=2.93, P<0.001). Conclusions: The findings of this study suggested that younger females with worse functional class require concern regarding the risk of developing depression. Regular depression screening and observing of NT-proBNP could be beneficial for early detection of depression in adults with CHD.
